Hola mi fiel lectores :)
BIRM - una medicine derivada de una plante de los andes... medicina indigina en el presente mundo...
Este es un producto ecuatoriano que una de mis profesoras me digieron sobre. Es interesante porque el medico que lo descrubio este producto, dice que tiene beneficios para todos los enfermedades, como cancer, diabetes etc... pero la unica estudio que yo podria encontrar era este:

pharm visits - vendedores de productos farmacéuticos
Hola Lectores!
Hay una cosa que no es un sopresa, pero es un poco preocupante a mi. Este es los vendedores de productos farmacéuticos. Hay muchos visitas a todos los doctores aqui de estes personas. La samples que darles a los doctores son muchos, y hay 3-4 visitas cada dia.
No me gusta este costumbre, pero no hay una movimiento por los medicos o estudiantes de medicina aqui como en los EEUU a ir contra este costumbre...
Tal vez la differencia es por la mas alta nivel de pobreza aqui, aun que entre los medicos tambien... no estoy segura... y no se que a hacer sobre este. Por ahora, pienso que la unica cosa que puedo hacer es no aceptara las cosas cuando estan offerados a mi...

Este semana pasada, yo estaba trabajando en un clinica de planificacion familiar. Mas o menos es una clinica de obstetrico/ginelogico. Muy interesante. Este clinica es privada, pero tiene precios mas bajos que los otros privadas en la ciudad. - This clinic is a private one, but the prices are much lower than most of the private clinics in the city.
La semana pasada era muy interesante porque casi el primero caso que you tuve era un caso de violacion - almost the first case I had was a rape case. La chica era un estudiante de medicina en su primero ano. Su historia era que ella salio con sus amigas y con pocos chicos quien no los conozco muy bien. Los chicos compraron algunos bebidas para ella y sus amigas, y despues de esta, la ultima cosa que ella recuerdaba era que despierto en una apartamento sin sus ropas interiores.
Cuando hicimos el examen fisico, estaba senas de violacion. La chica estaba llorando y era muy dificil y triste. La dra recomiendo que la chica visitara un psychologico despues, y hicimos todos los pruebas de enfermedades de transmicion sexual.
La chica no quisiera a denunciarlo a los chicos, y o supe que era muy muy duro para ella.
Despues la Dra me dijo que hay 3-4 instancias de casos similares a este cada ano. Y es muy comun que las chicas no quiere denunciarle a los hombres.
Era muy muy triste...
Mas sobre mi experiencia en este clinica este semana, pero, eso es todo por hoy.

La Familia
Querida Lectores,
Aqui en Ecuador, la familia es muy importante. Esasi en todos los paises de Latino America. Es Muy importante pasar mucho tiempo con su familia. Tambien las familias son muy grandes muchas veces. No les importa si eres familia politica, familia de sangre, o familia de cambio (como una estudiante quien solamente esta visitando por pocos meses =), todavia, familia es familia y es muy importante.
Cuando mis padres de los EEUU visiten, a mi familia Ecuatoriana, desea que ellos (mis padres), les visiten y pasen tiempo con ellos (mi familia Ecuatoriana). Nos invitan a ir a Otavalo y a cenar con ellos.
Porque mi mama es Puertoriquena, la familia es importante a ella tambien. Para mi y mi madre es una costumbre llevar regalaos cuando visitamos a su familia.
Para mi, me encanta este aspecto de la cultura de Latino America, porque pienso que la familia, en realidad es muy importante, por supuesto, mi familia es una (si no la unica) de los mas importantes cosas de mi vida. Yo se que mi familia va a estan presente cuandoquiera y les necesite.

El Crater, Cuy y Correa...
Querida Lectores!
Aqui son dos fotos de mi viaje a el crater. Uno foto es el crater de Pululahua, y el otro foto es de cuy, un plata muy tradicional de Ecuador y Peru!
Y un poco mas sobre el gobierno de Ecuador.... Hay muchas coasas beunas en la neuva constitucion, pero tambien, hay muchas coasas malas. Primero
algunos buenos coasas; el programa nacional de vacunas ha hecho posible para casi todos, ricos o pobres, recibir vacunas cuando estan creciendo. La nueva constitucion esta muy a loa izqueirda, y por eso, hay muchos ideales beunos. Muchas leyes que feuron escrtitas con la esperanza que fueran a ayudar al pobre. Pero, hay un gran rblema con estas leyes.... Ecuador, como muchos de los paises en Sur y Centro America, es muy pobre, y sin dinero para enforcar las leyes, estas leyes nunca significarian nada.
Muchos paises en Sur y Centro America han tenido que a las revoluciones populares, y despues
en muchos paises que se han formado gobierno neo-liberales. Es casi imposible explicexplicar el neo-liberalismo con pocos palabras, pero con el neo liberalismo, la idea de socialismo existe pero muchos veces los pobres todavia no tienen muchos derechos, y han bajado en la sociedad, todavia no les han ayudado.
Yo creo que neo-liberalismo no es el caso en Ecuador, pero, hay muchos caracteristicos que son similares. Este es un mal ejemplo de la constitucion, es que sin recursos las leyes no pueden ser forzadas:
En El Comercio de este fin de semana pasado, Correa le dijo a la policia que necesita parar a la delincuencia. Pero esa orden es ridicula, porque la policia ya esta haciendo todo que es posible con el dinero y recursos que tiene.
Por cierto, yo creo que ditribucion dela plata, por lo menos a la extente que todos tiene derechos basicos, es un buen idea. Pero, cuando hay poco dinero en total y un pequeno porcentaje con todo el dinero tmabien tienen mucho poder, a veces esto no es posible..

Pienso que es muy appropriado a escribir sobre dia de los muertos hoy... Este semana es un muy triste semana para mi familia ... mi hermana se murio este semana... siguiente su hija, mi sobrina quien se murio tambien uno y media años pasados...

A bit more on teen pregnancy here in Quito... 1 in 4, or 26% apprx of all pregnancies here are teen pregnancies (compare that to less than 10% in almost all of the developed world)... Given, here in Ecuador (and Bolivia) exist some of the highest rates in South America, but many of the other countries have rates that range from 15 to 20%...
Another interesting fact about the hospitals in Ecuador - Only public hospitals keep statistics on patients!! Can you imagine... the private hospitals and many of the employee hospitals, do not have any statistics on their patients or patient outcomes! que cosa! no?

Hola Mi Padres... y solo lector :),Esta semana, estoy trabajando en el hospital de Gineco Obstetrico, Isidro Ayora. Es un hospital publico, como el hospital de los niños. Aqui en Quito, hay 3 tipos de hospitales. Uno son los hospitales public
os. Estes hospitales son para los pobres, o la gente que no tienen dinero. Segundo, los hospitales seguros, hospitales que son para la gente quien tienen seguro via sus trabajos. La tercera son los hospitales privadas, estes son para la gente quien tienen mucho dinero.La parte del hospital maternidad en que yo estoy trabajando este semana, es solamente para los adolescentes quien estan embarazadas. Como el hospital para los niños, el hospita
l marternidad es muy diferente como los hospitales en los EEUU. En cada post parto sala, hay 8 madres adolescentes con sus bebes. No hay nunco barreras entre las camas, y no hay nunco mucho privadoso.En la consulta externa, los sitas de ante parte, son muy differente tambien. Se usan una "cone" de metalica para escuchar a la corazon de la bebe, casi nunca se usan la "ultra sound". Tambien, casi nunca hacen los examenes de sangre que son las costumbres en todos los clinicas en los EEUU durante los sitas de embarazadas. Solamente hacen examenes cuando hay un problema.
Hay cosas que son la mismo tambien. Siempre medidan la alta de utero, siempre se escuchan a la corazon de bebe, siempre se la prescriban multivitaminas...
Hay muchas cosas que yo todavia necesito aprender... pero, como un doctor me digo, todo es un processo!

Free School! Escuela Gratis and other things about the new consitution of Ecuador....

One very interesting thing about Ecuador that I have not yet written about is the new constitution... The brain child of R. Correa =) Here in Ecuador it is mandatory to vote! Everyone has to! if you dont you are denied an identification card, without which it is practically impossible to do anything! Incredible no?
The nueva constitition was voted into law shortly before I arrived, and it was a HUGE deal! The brain child of the current president, Correa, there are many very liberal tendencies in the new constitution; just as with most things in politics, there are some good things, some bad things and some neutral things in the new constituion. Interestingly, the approval rating among Ecuatorians for the current president is just about 70% pro, and 30% against.
One of the changes this new consitution has brought about is the institution of free public schools. While overall, this is a very good thing, it has made admission into public schools much more competitive. Medical School is slightly different here as well, compared to medical school in the states. Here medical students take entrance exams for medical schools right after college, it is very competitive, the public schools (Central) which cost much less, are much more competitive than the private schools (San Francisco)... many of the students in the private schools come from upper class families and unfortunately sometimes have a tendency to look down on the students in the public schools... clinical
rotations are shared and pubilc and private school students rotate though all the hospitals together.
Another things the new constituion tries to address is higher standards for medication control. Here anyone can buy antibiotics without a perscription (in addition to many other medications which require a perscription in the states, like sleeping medications), with the insitution of the new constitution this will supposedly be changing as well...

One point that struck me this week, was that in the US, between 2/3 and 3/4 of the population are overweight, in Ecuador, approximately the same percent of the population live below the poverty line, which, in Ecuador, is lower than a living wage.
It really depends on who you ask, but I believe there are many good things in the new constitution... I suppose we will just have to wait and see... time will tell =)

Hoy fue mi primero dia al hospital de los ninos. Today was my first day at the pediatric hospital. In the very short time that I spent at the hospital this morning (~3 hours) two abandoned infants were brought onto the general pediatrics floor. Infant abandonment is a big problem here.
Many factors contribute to this problem. Part of the problem is the huge percentage of people who are too poor to raise a child, and do not know enough about contraception to avoid having kids at very early ages - teen pregnancy is very frequent here. Abortion is illegal here and even when it can be obtained, it is often expensive and dangerous.
Aside of child abandonment, respiratory infection and gastritis seem to be prevelant ailments on the general pediatrics floor...
Baca Ortiz is the largest pediatric hospital in the region with the most available specialties... Four children inhabit most of the rooms, with no dividers between beds. Charts are completely paper. Blood to be sent for labwork is often taped to the wall abo

Hay muchos personas en pobreza aqui. 61.3% of the total population lives below the poverty line! The percent is much higher among people living in the country.
Although there are many people who would like to change this situation (including the current left leaning president Correa), there is currently no easy solution.
The situation unfortunately is similar in many South and Central American countries. Some are better than others, but most hae populations in which the majority of people live below the
poverty line. Many of the poor live in the country side, with indigenous and dark skinned people having the highest percentages of people living below the poverty line (close to 100% in many areas) and lighter skinned people living in the cities having the lowest (but still up to 20-40% in many of the cities)

Last night at 7:30, all of the tv channels and radio stations abruptly stopped what they had been previously broadcasting, and after several seconds of static - on came the soccer match! Ecuador vs Venezuela...
Aqui en Ecuador futbol es una cosa muy emotionada!! Here in Ecuador soccer is something very exciting! A source of much pride for many because los jugadores son muy bueno, y el ano pasada ganaron la copa - - the players are very good and last year won the cup!

It's the rainy season here in Quito!
Ayer llovio mucho! y tuvo mucho frio! Yesterday, it rained so much and was so cold! I had food poisoning from eating in a chinese restaurant on the way back form the beach :-P ick!
Pero hoy, hace sol!! But today it's sunny! Y me sentio mejor - and I feel much better!
Apparently, it is not such a good idea to eat at chinese restaurants here in Quito... :)

La Playa
Hola sola lectora,
Este fin de semana, fui a la playa, a un pueblo se llama Cojimies (mas sur de PEDERNALES). This weekend, I went to the beach, to a town called Cojimies :) The beach was so beautiful and the people there son muy 
buena gente! Tan simpatico.
The houses in the pueblos outside of the city, en el campo, are much smaller and the houses much less refined, much of the time they consisted only of three walls and a roof made entirely of sticks (not sure if this is exactly the co
rrect word, but you can see in some of the pictures... ) Many houses are cement, but of course none have insulation, windows etc and many are lacking at least one wall... something that is possible in such a warm climate.
Cows, donkeys horses and dogs run along many of the unfin
ished and roads, and often provide an obstacle for the already occupied driver. Often on two way roads, there is only one lane, causing cars to constantly swerve onto the side of the road to 
avoid oncoming traffic.
Driving through the mountains was tan bellisimo tambien.

Yesterday I went to the museum of Guayasamin - Ayer, fui al museo de
Guayasamin. Guayasamin is a world famous Ecuadorian artist, and his paintings (as well as the museum) were amazing (pix to the side).
Ok - Bueno, a couple of funny observations about Quito:

- McDonalds is super popular! People have bumper stickers! and you know how the slogan is "I'm loving it" in English - yep you guessed it, "Me Encanta"!! En Espanol! woah!
- Hay muchos perritos - There are many cute little dogs all over the city! but unlike many strays in the US (EEUU), casi todo - almost all of the strays here seem to be nice! how funny! :)
- La Hora Ecuatoriano - Time seems to have it's own meaning here... Oh, dinner starts at 7 you say?... wonderful! see you around 7:20 ish :) hehe, I love it!

la mejor analogía, creo que hay, es que la ciudad tiene la forma de una banana split. I live somewhere near the center (lengthwise) but at the top of the bowl (width wise). In other words, I live on the side of one of the mountains (hills? - colinas :) that borders the city. Which means, every morning on my walk to and from school (or other places) I get half an hour of beuatiful vistas! Who could ask for anything more? :)

Primero; el fin de Semana - En la fin de semana pasada, fui a algunos lugares famosas en la ciudad, incluido: 1) the water works building (pix above to the left :) - despite its unassuming name, is very impressive and in fact is the home to many social and political events of the city. Segundo; Escuela Ecuatoriano - Today is the second day of classes - Hoy es mi segunda dia de classes - and my professors are wonderful! - y mi profesores son maravillosos! Todos los dias tengo classes des de 8:30 hasta 3 de la tarde. Despues, voy a mi casa para almuerzo con mi familia! (after my classes, I walk to my house for lunch with my family). Me gusta mucho caminando en la ciudad, porque puedo ver mucho de la ciudad y muchos cosas interesantes.Tengo pocos temas para hoy! I have several topics for today :)
Primero; el fin de Semana - En la fin de semana pasada, fui a algunos lugares famosas en la ciudad, incluido: 1) the water works building (pix above to the left :) - despite its unassuming name, is very impressive and in fact is the home to many social and political events of the city. la parque Carolina which I pass on my walk to school cada dia :)Tercera; El Presidente de Ecuador - El presidente de Ecuador se llama Correa... Hmm... so much to say about this! ... I have been learning a lot about the political situation in Ecuador, so interesting! But am finding the more I learn, the less I seem to know! :) Entonces, creo podemos hablar mas sobre este tema en un otro dia... ;)
